Any work undertaken will only be started once I have your informed consent. 

This is an integral part of the BASES code of conduct and will be discussed during our initial session.

If you are an adult requesting a consultation for yourself please download the "Consent form for adults", sign and return to me.

If you are seeking consultation for your child then please sign and return both the "Consent form for parents" and ask your child to sign the "Consent form for children". Both should be returned to me.
